Thank you for inquiring about the Allergy & Intestinal Health Nutrition Services. You will receive an individualized and comprehensive nutrition booklet that answers your questions regarding:

The initial appointment (Step #1 and #2) is $60.00. Each follow-up appointment (Step #3) is $20. Prices include tax and are in Canadian dollars.

Payment can be made by check and sent through the regular mail.

Step #1 Gathering Information

A)      Initial Questionnaire
A questionnaire will be sent to you by e-mail. You complete and return it.

B)      Follow-up Questionnaire
Additional questionnaires may be sent to clarify the information in the initial questionnaire.

C)      Counseling  Summary Sheet
This sheet summarizes your concerns, goals (what you want to accomplish) and plan (how are we going to accomplish the goals). This sheet will be sent to you, and you can give feedback

Step #2 Answering Your Questions/Providing Information

A)    Comprehensive Package of Information
I will send you a comprehensive booklet of information to answer your questions. This will include detailed menu plans, recipes and shopping information.

B)    Telephone or Personal Appointment
We will arrange an appointment (in-person or telephone) to review the information and answer additional questions.

Step #3 Follow-Up Appointment

We review the progress that has been made on the “Counseling Summary Sheet”. Follow-up appointments are usually conducted by telephone.
